ive done all that Rene , they have the letter and confirmed it was scanned onto my patient record.

My email stating I would consider going to the information commisoner and health board seems to of done the trick as I had emails stating should be ready by end of the day

I went direct to my GP and asked regards the medical summary, he then told me to go to reception to fill out a request form which I did, after 2 weeks I had what I needed.

If you wish to apply for your full medical history (I did as I wanted it to take with me if accepted to the US and also for any reports on operations etc I may need for the medical) I wrote this letter and handed it personally to my GP, I got the records in around 2 weeks and did not have to pay for them.

Please will you provide a copy of my medical records as I am entitled to have under the Data Protection Act 1998. I wish to the have the records made during the period (start date) to (end date). Please will you provide the copies within 40 days of receipt of this letter. I require the records due to currently applying to immigrate to the United States of America and will need them for my medical examination.
