It's based on line 22 of the 1040. Although in at least one circumstance where the Other Income of line 21 is a negative you can use the sum of lines 7-20 instead (line 22 is the sum of 7-21).

From the INS Fact Sheet for Affidavit of Support, Form I-864:
18. Which income line from the federal tax return is used for income determination?
The line for gross (total) income on IRS Forms 1040 (line 22 in 1996) and 1040A (line 14 in 1996) is used to determine income. The line for adjusted gross income is used for persons filing IRS Form 1040 EZ (line 4 in 1996).
On the Form 1040, line 22 hasn't changed from 1996 to 2000. Sorry for the confusing extra explanation that's probably not applicable to your situation. :-)
