Default Free public healthcare for British residents covered by the WA

British nationals who entered and have been living in Italy prior to 1 January 2021 and are beneficiaries of the Withdrawal Agreement can register for free (iscrizione obbligatoria) with the Italian National Health System (Sistema Sanitario Nazionale) and are therefore not required to make voluntary contributions.

You might ask for a refund. May be they will make a tax rebate next year?

Nor me. Just spent a month reregistering with the NHS, because the problems we were having with our local ASL being a factor (but not the only factor) in our decision to move back to the UK. Ah... the irony! Still, I'm pleased for everyone else. Also because the statement from the Italian govt includes a couple of very clear statements about the rights of UK citizens in Italy as a result of the WA , which might be a useful precedent for the future in other situations (not just healthcare)
