x-ray's part of forms missing

Hi All
looking for info for x-rays and med's
we are filling in the forms for the x-ray and it has told us to fill in part A & E however there is no part E we downloaded the forms from Aus house

for the med's it says fill in page 10 but again there is no page 10
is there something i have missed ????
