stni vic approval/acceptance form

i am looking for the stni vic approval/acceptance form but am unable to find it.

Can anyone direct me to the link please

That's strange I'll copy the link again, you just scroll down abit and it allows you to download the application form in word

Nope it won't let me go in once I posted the link. Just scroll down to the bit where it says "Skilled Migration Visas" on the left then on the left again scroll down to the bit where it says " How do I apply" and it takes you into the page where yo can download the word document!
