which form is required? please help its URGENT!!!!!!!!!!

which form is required? please help its URGENT!!!!!!!!!!

Hubby and toddler are off to have their med checks and xray tomorrow
but I haven't been able to find the forms can anyone help? We live in
Italy at least two hours from any Aus. consulate/mission.
Can I find these on the DIMIA website? URGENT

Hi,
If you go to DIMIA website you can download Forms 160 for the x ray
and Form 26 for medical examinations,good luck,steve.
