hey ,
i was checking the thames valley police website. i am confused about which form i should fill to get my PCC? IS IT SUBJECT ACCESS APPLICATION FORM? and should i send my original identification documents or solicitor attested will be accepted? thanks for your replies

Hi

Yes it is the subject access form. You need to send an original document as proff of ID. I sent my birth certificate and they returned it 2 days later (Thames Valley don't allow you to take your application in and have docs witnessed, they have to be posted). You could always sent it registered post if you were worried about the document getting lost.
