questions about Name Changes, stepfather, half-brothers and sisters

Default questions about Name Changes, stepfather, half-brothers and sisters

I have a couple of questions for the independent immigration to
Canada. I searched this newsgroup, but did not get conclusive answers.
Therefore I am asking for help from you.

I am the principle applicant. The questions are related to my spouse.

The background information:
She had NAME1 when she was born.
Later, her parents divorced. She lived with her mother. Then she had a
second name.
Her had NAME3 10 years later after her mother re-married. Her mother
and stepfather had a son.
On the other hand, her natural father married again and had a
daughter. My wife rarely visits his natural father. In fact, she lost
contact with hime for the past 10 years.


Now my questions are:
In Form IMM 0008, Schedule 1
1.Should she put NAME1 & NAME2 in item #3: "Other names your are or
have been using (including name at birth, previous married names,
aliases)" ?

2.We have to provide school documents, work experience and letter of
reference by her NAME2. How to correlate her NAME2 to her current
name, NAME3?


3.What supporting material should we submit to the Canadian
Immigration Authority for the name changes? Although she changed name
twice, but she does not hold any document for the changing since no
documents were issued in the first place. Instead, the police station
keeps all the records. Should she get a report from the police
station? Should the report get notarized?

4. When asked for "personal details of your father" in item 7, should
she put his stepfather or natural father in there?


In another form, "Additional Family Information"
1. Again, in Section A, should she put her stepfather or her natural
father's name?

2. What is the definition for "half- and step-brothers and sisters" as
mentioned in Section C;Should she include the name of the daughter in
his father's 2nd marriage even though she rarely visits her natural
father after her parents divorced?

I will appreciate your reading and answering my questions.

When I applied I had 3 names to list. Although my first and middle name were always the same, I had 3 last names: birth name, adoptive name, and married name. I included my birth certificate with the original birth name, my adoption papers with the new adoptive name (adopted at 1 year old by grandparents), and my marriage certificate. I had used the 3rd name for almost 20 years since I was married at 19 and had been a widow for 4 years when my application process started. I also included my late husband's death certificate too just to tie up the name chain neatly. Since I am now married again, I have added that marriage certificate to the name chain and had to give it all again when I applied for my PR card.

When I filled out family info, I used my adoptive father and mother's info in personal details section. If your wife was adopted by her step father, then that is what I would do for her too since he would legally be her father. If he didn't, then I would expect it is the legal father's info she needs to include. The step father when then go in the extended family details. I would say any children her father had in a later marriage would have to be listed as half siblings since they are by blood. Her family details are going to get long, but I would list everyone to be sure.
