Default Mr. Miller, PMM or Jim - just a quick question

I know I'm nagging but I just don't want to screw it at the end of the line. When I'm picking up my fiance's passport with visa at Buffalo - how should we prepare the written authorization? Do we need a Commissioner of Oaths?
