Missed communication -Med request

Missed communication -Med request
Hi, could anyone please answer my query,
I have received a letter today from CHC stating that on 18/04/08 a letter was sent to request for Medical Examination and Right of permanent Residence fee, this letter also states that I have to submit these two documents in 30 days time.
For medical examination the letter states that Instructions & forms were enclosed with the previous request letter.
I have never received that letter, so my query is do i need to contact CHC and ask them to resend the medical forms or can i print them from the website on my own and take them to a doctor.
Could you please let me know as soon as possible.

The medical request document is specific to you not a generic downloadable document. You will need to get in touch with CHC asap. Good luck.
