I have two children under 16 years old. I need to fill up Immunization
record form and certify by a qualified health professional. At the end
of form, it ask information about health professional such as:
"SIGNATURE OF QUALIFIED HEALTH PROFESSIONAL", "DATE","NAME","ADDRESS"
and "PROFESSIONAL DESIGNATION".
There is a fields that confuse me. I don't know what to fill in
"PROFESSIONAL DESIGNATION" field.

Any ideia. Thank you.

Either a Dr. or if the immunisations were done by Nurse practitioner.
