When should we book flights?

When should we book flights?
Should we wait until our son's visa has been granted before we start looking at confirming dates? The rest of us have ours and his medical was sent off a week or so ago. We received confirmation of ours only 8 days after the x-rays arrived, so I'm hoping we'll hear whether we need to leave him with the neighbour, sometime very soon. 
With 2 adults, 4 kids and a 9 month old, is there any rhyme or reason to when you book flights? As soon as visas are confirmed? A month in advance? etc, etc.

A quick answer as not sure of your situ obviously. Probably best to wait till everything else is done and dusted - we booked ours abt 6 weeks in advance, as did our friends who are coming over next week

We booked and paid for our flights in our last week courtesy of Last Minute. com.
