New Passport ..
Hello..
My passport (contains 136 subclass visa which I had received last year) will be expiring soon. Can anybody advise on which form I must submit to inform DIC on this change so that I can get the visa on my new passport.

You don't need to submit any form. Just take both old and new passports to any DIAC office and they'll give you a new visa sticker.
