It is not available for download, you can only request it either direct from Adelaide or from Australian Embassy.

when you get through to the medical stage, adelaide will send you the required forms (as we found out last night!!!)

Congrats flying dutchman did u get email confirmation that they were on the way or did they just turn up

we had heard nothing since our acknowledgement in July last year. I was expecting them to arrive sometime in March, but was a bit worried about our proof of job experience etc. in the end they have approved the lot, including our relationship , which at the time of applying, was only just over a year old!
the envelope just appeared in the post box for me to find when i got home!
