Default Format of Reference letter for skills assesment from ACS

Hi,

I want to send my documents to ACS for skills assessment.

I have the following two things to ask:

(1) My university transcripts show the course grades and in some courses my grades are not good. So does it matter? Is ACS going to take into consideration these grades during assessment?

(2)What should be the format of reference letter from an employer. Is there some website showing some modal reference letters?

Currently, I have the following skeleton in my head of such a letter. Do you think this should be ok?

TESTIMONIAL
=========
Name: ...
working period: ...
Assignments: ... (short descriptions of tasks I carried out)
Tools Used: .... (e.g., C/C++, UNIX platform..)
Evalutaion of the person: ...

All I sent them was

Name :
Position/Title :
Dates worked there : to/from.
General Duties :

Hi,

I had bad grades in some courses and it didn't prevent the ACS from granting me a
positive assesment. I suppose many students have bad courses.

As for a format - I haven't used a "form" like the one you propose but your looks ok,
as long as it contains all the information asked in the ACS guidelines.

All my employers asked me to write the letter and they just checked, printed it (on
company letterhead) and signed 4 copies for me.

Here is a letter from one of my employers:

To whom it may concern

Amos Shapira was a full-time employee of <FULL COMPANY NAME> since <EXACT DATE> until
<EXACT DATE>.

His role consists of analysing, designing and implementing web applications which
allow users to manage and modify XML files through a web browser.

In is role, Amos demonstrated a high level of knowledge of Java, Java Servlets
and JSP. The platforms used for the product are Microsoft Windows 2000 and Sun
Sparc Solaris.

Amos’ part in the project includes analyzing, designing and implementing
interfaces for manipulation of XML files, infrastructure for Java Servlets and JSP,
system-level file-locking.
