1. All transfers are reported at Lines 5a and 5b on page 1 of Form 1040. If you are a resident of a State which has an income tax, the distribution may be taxable in that State.
2. Both the old and new plans are reported on FBARs and Form 8938.
3. If the SIPP is a foreign grantor trust, you'd be filing Form 3520 and a substitute Form 3520-A.

Will I need to declare uk state taxes somewhere on my US tax form and if so, where do I enter it.
This will be the only state pension that I will be receiving since I don't have enough years to qualify for social security but have the maximum UK years, 39 years. Not sure if that makes a difference.

The UK State pension is always reported at Lines 5a and 5b on page 1 of Form 1040.

On the question of 3520/3520-A, I was interested in this
r in which the tax lawyer seemed to say that while SIPPs are likely foreign trusts, they need not be reported on 3520/3520A and that the greatest risk would be to decide to start reporting by filing past due 3520/3520As as this would likely trigger penalty notices.

In TurboTax 99-099999 works as the payer tax ID for the substitute 1099-R

Many with SIPPs do not file Form 3520. The IRS has never made a definitive statement regarding the need for this form in relation to a SIPP, they have never fined or penalized anyone for not filing in relation to a SIPP, and it is not the easiest form to complete (otherwise we would all just do it). Personally I wouldn’t worry about it.
