We didn’t have a PCP (still haven’t managed to get one nearly 2 years later!) but we found an urgent care centre that would do it.

I think it will vary from state-to-state but when we moved we had our doctor in Hong Kong fill out our first vaccination record (using the state form from our school). It wasn't a problem.
Later our primary care physician in the USA took the HK doctor's work and put it in their system and they filled it out going forward.
Interestingly, there was a typo in one of the staggered vaccinations (1-3-6 months apart) and it got caught by the government's public health department. They called us and said we hadn't had the 3rd shot properly spaced and it wasn't valid, but I was able to fix the date with the help of some old medical records. So basically--they did check the details pretty thoroughly.
