If they have an Irish (including Northern Irish) parent or grand parent, they are, I think, eligible for an Irish passport, which could get them into the lottery, couldn't it?

Correct. My ex wife was born in Bolton to NI patents. She was entitled to apply for an Irish passport and entered the green card lottery some years back.

I would have thought the best bet would for her to get the transfer and you to piggyback.

Getting sponsored as a non qualified Dentist seems a long shot.

Getting sponsored as a non qualified Dentist seems a long shot.
And qualifying on a student visa with no guarantee of getting a working visa is expensive and risky; possible, likely even, with careful planning, but still risky.

Having an Irish passport doesn't mean you were born in that country, though, right? For example, my Iranian husband has a US passport, but he wasn't born in the USA.

Not EVEN residents, the nationality (evidenced by passport) is the defining qualification. It doesn't matter where a British citizen resides he/she is not eligible for the DV lottery (unless he/she has an additional nationality which is eligible.)

No. The DV lottery is open to people BORN in certain countries.

It does not matter where the person is a Citizen or Resident. What matters is the country where they were BORN.

There is some kind of rule where you can also qualify based on your grandparent. I am not sure of the details.
