The OP seems to want to live in the US, but commute for medical treatment.
I am sure many do, but it is probably pushing it.
Maintaining US residency as well may be another issue.
I wonder how you would go about explaining to the NHS your need to substantiate your medical condition to obrain a re-entry permit as you are US Resident?

There is a critter called a 'returning resident's visa' (SR-1?). I dont' know anything about it, but you might do some research.
What will you do if they bounce you at the border, under your plan?
