H1B extension after 6 years...(URGENT)

Hello Experts,
I want to find out if it is possible to have my H1B extended beyond the 6 year period if I have spent a few months during the 6 years outside US?
In any case, I shall be applying for an extension anyway. I would therefore also like to know how long it would take to hear back from the INS (on an average)regarding an acceptance/denial of my H1B extension.
Also please let me know if my extension application is denied, how much time does the INS give to leave US on the new I-94 it issues.
I am talking about the INS in Vermont.
