I am travelling to America this summer under my third J-1 visa in four years. Is it possible for me to extend this J-1
visa upto the maximum 18 month period, or must I apply for a separate 'extended J-1 visa' If the latter is the case, can I do it whilst in the US or must I return to England to put my case to the Embassy in London?
Any suggestions?

If the J-1 category is J-1 trainee, and if the J-1 sponsor permits it, yes, the J-1
trainee status can be extended up to the maximum duration of 18 months without your
leaving the U.S. J-1 status extensions are handled by J-1 sponsors, through the
issuance of an additional IAP-66. The Embassy handles only J-1 visa applications.

If your category is J-1 summer work/travel, or some other category, you would have to
leave the U.S. and return in J-1 trainee status. The State Department Exchange
Visitor Program almost never allows the change of J-1 category within the U.S.
