Extended J-1 visas

I am travelling to America this summer under my third J-1 visa in four years. Is it possible for me to extend this J-1
visa upto the maximum 18 month period, or must I apply for a separate 'extended J-1 visa' If the latter is the case, can I do it whilst in the US or must I return to England to put my case to the Embassy in London?
Any suggestions?
