Your 90 days started when you arrived in the USA ...
and ended when you returned to whence you came ...

penalty is that you will have major major problems entering the USA in the future, as their systems will show that you overstayed your visa. When you leave the USA, that green card in your passport is taken from you and submitted to immigration. The date that it is taken from you is the date entered as the date of your departure. If you want to stay longer, simply go to the nearest immigration office and extend it. Avoid future hassles of trying to enter the USA in the future.

I assume you did not get a new I-94 card when you re-entered the US from
Canada..

In that case, if you leave more than 90 days after your initial arrival, you
will not be able to travel visa-free to the US in the future.

They care. You would never again be able to use the visa waiver.

A stay under the Visa Waiver Program cannot be extended, because there is no
visa to extend.

You will not be able to use the Visa Waiver in future. You'll need a
visa to enter the USA in future, and you may have difficulty getting a
visa because of the overstay.

Yes.

You would have been better to have them take your I-94W when you went
to Canada, and apply for a new 90 days when you returned to the USA.
