I just became an United States Citizen and I would like to file for my
mother who is here in the US but she has been out of status for
several years. How do I go about this? Does she needs to leave the US
to file the paperwork since she is out of status? What forms do I need
to file? She is also currently living in NY and I am living in IN. Can
we file the petition in NY?

You would file both a petition and an application to adjust status at the
same time in the NY office. DO NOT allow her to leave the US, as she would
automatically be banned from returning for ten years despite your
relationship.
I would recommend you hire an immigration attorney to make sure things go
smoothly.
