I believe it is the I-485 form that asks for a copy of the passport page that has
the nonimmigrant visa in it. What if you came here on a Visa Waiver? Would you need
to include any copies of the passport with the AOS package other than the I-94W
form? Thanks!

Exactly as the form says: If you have a non-immigrant visa, give a photocopy. Since
you don't have one you just need to copy the I-94W form you have. BTW, I removed mine
from the passport before photocopying it.
