Default Vaccination supplement and TB test

My wife came from the Philippines years ago, to marry me, on a fiancee
visa. Our first application for her green card was denied by mistake,
and we kept trying to get it reopened, for years, then finally applied
again. At our interview for the 2nd green card application, the person
who interviewed us said we only needed the vaccination supplement, and
only because so much time had passed that she was due for a follow up
vaccination, because one of the vaccinations requires more than one shot,
over a period of several months or more, and we never got that. So now
we're trying to satisfy that requirement. The person who interviewed us
did not mention civil surgeons, but implied any doctor or county health
center could do it. But it has to be on the correct form, which is the
vaccination supplement.

The problem is that when we went to get the vaccination, the medical
office that did it refused to put all the information on the correct
form. They said they could only put the information for that one
vaccination they did, and could not copy the old information to that
form.

So we called the nearest civil surgeon on the list, and they're a long
drive from here. But they said on the phone that she needs a TB test.
Until then, nobody ever mentioned a TB test to us. She got an xray in
Manila as part of her medical to come here, and the doctor there used
that to determine she didn't have TB.

So now we're not sure what to do. Does she really need a TB test at this
point? The person who interviewed us didn't mention it, but just the
vaccination supplement. But how can we get the vaccination supplement
done without a civil surgeon? Even though the interviewer didn't mention
a civil surgeon, we can't seem to get the correct form completed
correctly without one.

If she has been vaccinated against TB, her TB test will come back positive.

Get another X-ray done before you go for the vax supplement.

But make sure they are only charging her for the vax supplement and not for a full medical.

If she needs any vaccinations, tests, x-rays, those can be done by any physician, and is probably cheaper through your own insurance.

When you have what you need, go to a USICS Civil Surgeon, present the proof that she's had all the vaccinations she needs, and he will complete the correct form. The Civil Surgeons have the form, you don't need to supply it.

The Civil Surgeon will give you a sealed envelope, which is what you will return to your officer (or wherever she said to return it). Do not open the sealed envelope. If you want a copy of what's inside it, ask the Civil Surgeon for your own copy before it gets put in the sealed envelope.

When calling around, it might be hard to convince a Civil Surgeon that you don't need a whole new medical. Make the appointment and tell him in person over and over again until he understands you are not there for a whole new medical, ONLY the vaccination supplement. It took some convincing on our part when we had to do this. Stick to your guns.
