Vaccination Supplement

Hi, We are currently preparing the I-485 packet to send to the INS in Atlanta. I am
on a K1 visa and have already had a medical at the US Embassy in London. From reading
the instructions on the back of the I-485 form it looks like I need to send in a
vaccination supplement.
Is this a separate form? How do I obtain it (i do not see it in the forms they have
sent me), do I need to visit one of the INS approved physicians to get this
supplement. I do have vaccination records... would these be acceptable to send.
Any advice/experience would be appreciated here
