URGENT: Need for for medical for exam
Does anybody know where (online) I can get the form that the doctor needs to fill out for the medical. (for a consulate immigrant visa case(if that matters)).

The form is an I-693, but since the medical exam has to be carried out by a
doctor designated by the consulate, I would expect he/she has all the
required forms.

u should get this form from your local BSIC office because this version is not printable and consists of 4 pages(4 copies)one for u, one for doctor, one for BCIS and one for someone elsse-don't really remember.

I DOWNLOADED MY FORM FROM THE BCSI WEB SITE. DON'T KNOW WHY THE LAST THREAD SAID THIS WAS NOT POSSIBLE.

I thought so, too, but it turns out that this form is actually available
online on the BCIS Web site.
Well, that applies for the BCIS version of the form - the version used by
consulate is different.
