Report failed K-1?

About a year ago I went through the K-1 process and my fiancée came here.
After coming here she decided she did not want to marry me and returned to her
home country.
Do I need to report this to the INS?
I now want to go through the K-1 process again.

Yes; you will get a chance in the new petition for the current fiancee.

On the front page of the I-129F ( bottom left), for question number 13, you can attach an
additional sheet and explain the case of your past fiancee.
