You need the Notice of Approval letter of the sponsoring H1-B holder. H1-B holder's
latest bank statements, salary slips, and sufficient proof of relationship as a
spouse or child of the sponsor, and a letter from the H1B to the consulate officer
requesting the H4. That should be sufficient.

    > * How many days does it take?
Typically it is issued on the same day. You have your interview in the morning, and
you can pickup your passport with the visa stamped in the evening.

You can also call the consulate to confirm the latest requirements. My friend's wife
came here on H4 some months ago from Mumbai, and she called a number of times to
clarify a lot of issues. Her experience was that they were extremely helpful &
friendly each time, so when she went there to actually apply, it was a breeze since
she had everything ready.
