what do i put in?

No, you do not have one.

You don't have a visa, so you don't have a consulate where it was issued.

You don't have a visa, so you don't have a date on the visa.

How about "none"?

also its says list your present wife etc... do i use her name given at birth under family name or her last name before she married me?

Well, did your present wife take your name? If so, that's her name...!

Actually I would put (and this is what I did)

Worked for me since they will see both her maiden name and new married name on the G325A she needs to complete for the I-130.

You do know that the I-485 is NOT the only documentation that needs to be filed. Right?

she hasn't changed her name yet she wants to keep it for a little while as all of her documents and degrees are under it, not to happy but i understand. she will change it with some nagging from me soon.. i'll use her name as it stands.. thanks again

That's what we did. I am the USC and I did not change my name right after marriage either. We filed all my husband's AOS forms in my maiden name, then a year or so later I went ahead and changed my name.

It's not a big deal.

Agreed. I'm the USC and I took my husband's name - we did not refer to my maiden name on the I 485 and it wasn't a problem. (Obviously my maiden name was on the marriage certificate we enclosed.) So you can definitely do it either way.

Hmm... another MCP, I see. My wife not only kept her name from before she married me, she kept her 1st husband's name - because that was the name under which she received all her degrees and professional certifications. Being comfortably secure emotionally, I have never had a problem with her decision. I suggest you question your motives for wanting her to change her last name to yours.
