Medical with AOS to Charlotte or not?

Medical with AOS to Charlotte or not?
The rules seem to change monthly, it is required now to send the medical with the 485, or wait for the interview? It would help to save the money on this until later as for a family, it quickly becomes that months mortgage payment for a useless paper formality. And if they lose it, or they go over the year that the medical is good for, can they force you to pay for another at your expense???
