Lost I-94

Lost I-94
Hello,
i'm about to begin my I-485 / 130 process and have discovered i cannot find my I-94 card that was stapled to my passport, it says i need to enter it on one of the forms.
What can i do?
Will it affect the process?
I know when i entered and was due to leave the u.s. (march 13th 2002) and that i am substantially over my period of stay.
What is my next step, can i leave it blank, or write "lost"
Please help.

The fee for this was $100, but was raised (I don't know to what). Good luck!
