J-1 travel - IAP-66 question

Hi,
I am going home for a month during the winter break. I am a graduate student in J-1
status and my current IAP-66 won't expire until 2003. However, I will need to get a
new visa stamp back home.
I took the pink copy of my IAP-66 to the international student office of my
university for them to sign on the back. However, in addition to the signature on the
back of my current IAP-66, they also issued me a new (3-sheet of white-yellow-pink
set) IAP-66 that was marked "amend a previous IAP-66 form". The new form has the same
expiration date in the year 2003 as the current form. However, the new form does not
have a signature on the back.
The advisor was not there to answer the questions when I was picking up the forms and
it's Thanksgiving break. So:
1) Which form do I use to obtain a new visa stamp?
2) Do I need to take the new 3-sheet form back to the international student office
for them to sign on the back before travel? My current (old single pink) sheet
already has the signature on the back.

Have the new IAP-66 signed by student advisor before you leave.
Regarding visa, give the new one. Most likely they will want to see the original one.
On return trip, keep both the old and new IAP-66 ready. They will want to check both.
Looks like, your old IAP-66 has not expired. Hence, you may need to provide that
document, if required.
I just know of a case long ago when the visa was extended based on the new IAP-66,
however, at entry point, the original IAP-66 was asked. ence, keep both documents
handy which will help you.
