I485 Medical Exam Questions

I will be ready to file for AOS soon. I do not have my immunization records. At the time
of filing for AOS, I will be undergoing medical tests. I have following questions:
1. What do I do if I do not have the immunization record.
2. In that case, would I need to take all the vaccines for various diseases?
3. The vaccine for Hepatitis B has to be taken in three installments, how will
this be done?
4. It might be dangerous to get so many vaccines in a short period of time.
5. Suppose that someone got several vaccines in the past (with no record), will the doctor
(civil surgeon) be required to check if this person really got those, since applicant
may not exactly recall?

If you have more info. or underwent this process already, then please share your
experience in this matter.

I did the medical examination late last year. I did not have any immunization records, but
I did recall having a skin test for tuberculosis about 10 years ago and told the doctor
about it. I ended up having only two shots (I was expecting more since the list of INS
required immunizations was so long). I think each shot had two vaccines. Hepatitis was not
one of them. For me it was a pretty simple process, I had no side-effects afterwards,
apart from a sore arm for a few days. This was a clinic in Sunnyvale, CA.

Now the blood test was another story... they could not find the vein and the blood would
not flow... they tried several needles in each arm... it was very unpleasant.

If you don't have any imunization record, you'll get your first installement during the
medical visit and you pay for them

Yo don't need all installments. You need to be current. Which means that if you had your
first shot today and the next one is schedulled in three months you are current. .

I had 5. I didn't need them all, but it was more complicated to prove I had imunity. I
felt kind of dizy for the rest of the day, probably my body had a strong reaction to one
of them. The nurse said it was safe.

In this situation my question was "what is faster - re-taking the shot or searching for a
lost record?" I decided to take the shot.

Sanjay,

As ppl have already replied to you, you'll have to get a few shots again. That's always
safer than trying to search for old vaccination records.

Also, which shots you take will depend on how picky your doctor
is. I had virtually no records, but only had to do three-four (MMR, tuberculin skin test,
diphtheria, tetanus).

I had hepatitis when in India, so I was automatically exempt. Can't answer that one.

As for getting sick, again, YMMV. My experience was awful. I had all the above on the same
day. As the doctor predicted, I got a mild pseudo-case of mumps two weeks afterwards,
swollen tonsils, chills, aching joints, fever, cold, the works. The viral infection
passed, but was followed a week afterwards by a severe (bacterial) sinus infection from
the residual phlegm in my system. From start to finish, it took one month out of my
schedule; I lost 10 pounds in the process (I am very fit and don't really have 10 extra
pounds to lose!); and being in general a healthy and physically active person, I was
totally amazed by the extent of the sickness. The doctor said that since I had already had
both MMR shots as a kid, perhaps there was some crazy interference going on between those
older antibodies and these newly developed ones.

Looking on the bright side, my immune system is stronger than ever now!



So you may want to plan ahead: if you are taking MMR, make sure you can take some time off
two weeks afterwards, b/c that's when you may get sick.

Did they request any proof? My wife did not want to have the MMR because she had natural
imunity. She had to do blood tests to prove it. Finally it was longer, more expensive (in
case the test was not covered by insurance) and more painful than simply having the shot.

OTOH, the hepatitis shot is for hepatitis B. Are you sure you had hepatitis B in India? If
you had hepatitis A (which is more common) you are not imune to hepatitis
B.

The decisions about all that are up to the individual doctor administering the exam. If
they are medically inadvisable (for instance, due to an egg allergy or simply because too
he believes that too many immunizations are dangerous) or if there are religious reasons,
the doctor can waive any immunization requirement and still give you a medical
certificate.

The doctor may also chose what kind of evidence to accept of prior immunizations.
