I-94 form
I'm Canadian Permanent Resident from one and a half year. I came to
Canada from US where I was with Work Permit. When I entered in Canada
I didn't surrender mi I-94 form, because I couldn't find where I have
to. Also when I entered in Canada my I-94 wasn't expired, but now it
is. Now I'd like to apply for Visitor visa to USA. Can anybody tell me
more information about my chances in getting it? I will appreciate any
suggestions. Thanks.

mail I-94 to US consulate with a letter or copy of boarding pass when you
left US
you shudnt have a problem with the visa if you have a job, home
lease/purchase. other bla bla in canada etc/

There is a way to mail it in with evidence that you departed on time, but the address is somewhere in Kentucky. There is actually a procedure for it but I can't seem to find it on the net.
Your landed papers should also have a date on them indicating when you departed.
