I-94 and DS-157

I-94 and DS-157
Hi,
I reside in the US. Assuming I got my Notice of Approval I-797 for an
H1B visa.
If I go to my country of birth (France), I need to go to the Consular
section of the US embassy in Paris to get the visa sticker sticked on
my passport, so that I can re-enter the US.
1) Am I supposed to provide my old I-94, and the new I-94, sent by the
BCIS with the I-797 ? My problem is that the two last times I left the
US for France (for vacations), the I-94 stapled on my passport was
removed at the US airport, by the airline staff, before boarding. Do I
need to make copies before?
2) My second question is : who needs to fill out DS-157 and DS-158
forms?

For your first question, you do NOT need to make a copy of the I-94, and,
just as you experienced, you would not have an I-94 when you are in Paris.
In fact, if you still do have the I-94 (it does happen that the airline
forgets to collect it), you should return it to the consulate. Otherwise,
BCBP thinks that you are still (and possibly illegally) in the USA.
As for the second question: male applicants aged between 16 and 45 need to
fill out the DS-157. The DS-158 is required for people applying for F, J
or M visas.
