I-693 Vaccine Supplemental??

Default I-693 Vaccine Supplemental??

My wife (who entered on a K-1 Visa) has just received notificaiton
regarding her AOS status (it has been almost 5 months since our
interview!!), that they require an I-693 SUPPLEMENTAL to be sent to them
before they approve her.

Since she entered the USA on a K-1 and we applied for AOS less than a
year since her entry, I knew she didn't need to have another full
examination. However, the Examiner at our interview never mentioned this
supplemental that this letter asks for.

Actually, I DO have proof that she had ALL the required vaccines as part
of that exam as required by the US Embassy in her country (and even
enclosed it in the paperwork at the interview--specifically the Form
DS-3025 Vaccination Documentation Worksheet which is required for ALL
U.S. Immigrant Visa Applicants)

My question: Since this will evidently NOT suffice for the AOS, has
anybody taken this form to a "Designated Civil Surgeon" and had them
simply "transfer" (or copy) the dates of the vaccines given (all given
within the last 14 months according to this form DS-3025) to this I-693
SUPPLEMENTAL??

I see NO reason why she should have to ( or want to??) take the same
vaccines AGAIN just a bit over a year since initially having them??

So, IF I take this form to a "designated" Civil Surgeon approved by my
local BCIS Office, what type of fees/ charges should I expect to pay?

And lastly, is there a LINK to find this form??? I want to see EXACTLY
what it looks like, and have it IN HAND to take to the local Doctor.

It has exactly the same information that is on the Vaccination Documentation
Worksheet form from the fiance medical form DS-3025.

The designated civil surgeons have this form already in their office all you need to do is show proof of the vaccinations, they will copy the informaiton over to the approved I-693 Vaccination Supplement form and you pay (in our case $30). They give you a copy for your records, they seal a copy for BCIS and then keep a copy for themselves.

It's not too painful. Just call around when you are looking for a civil surgeon the charge for this service varies widely...

That is the exact form that the doc here just wrote up for me for my fiancee
visa medical ... and I have a copy of it ... Does that mean I don't need to
get another one done for AOS .. I can just take that one?

You will still need to submit another copie, signed and sealed by the civil surgeon when it comes time for your AOS.

Hi Harley..
I found the "local" Designated Civil surgeons close to me and rang them asking if they will do an I693A only examination as I was a K1 entrant into the US. .One inisted that I needed the full medical at $250 so i called the other.. Explained (as does the BCIS web pages) that all i needed was the I693A completeing and that I had a full official copy of my vaccinations from my UK doctor on headed notepaper..
That was all they needed.. I took all the evidence with me and because I already had TB immunity natually when I reacted to the Heaf test as a youngster, I took my Xray that was part of the UK Embassy medical with me to prove I was TB free. The Doctor did just that, took the evidence from me, sat along side me and filled it all out transferring it almost word for word. My UK doctor was quite thorough and had written batch numbers of my various jabs too which helped I feel.
$30 later, I had a sealed BCIS copy for my AOS package to send to Arlington and a copy for myself.

HOWEVER, be aware that the BCIS interviewing officer, according to a BCIS officer on the help(?) line, can still insist on a full medical if he or she is having an off day :-) Its totally up to them.
