Default Help: Lost Passport - H1b Visa - Procedure/Documentation needed for restamping

Hi,

I have lost my passport,I-94 and H1b Visa stamp on it.

The visa is valid till Sep 2007. After I lost the passport I got a
duplicate passport and replacement I-94. I have informed the visa
issuing consulate (in my case chennai) about lost visa and they
acknowledged the same and have cancelled the lost visa.

Questions:

-> Can i apply for a new visa from within the USA
-> Can i get the visa stamped through canada or mexico if yes wt
documentation i need to carry?
-> In case I have to get visa stamped from my home country (india) -
will they consider this as a fresh application or would simply
revalidate the earlier visa and give a new one without many queries?
Also wonder wt documentation is needed to get visa stamped through my
home country.

Thanks in advance.

Regds.
Sanjeev.
