I have a couple of questions here which I would appreciate your help.
3. THE APPLICANT'S NATIVE COUNTRY IF DIFFERENT FROM COUNTRY OF BIRTH
- I am now living in the country I was born. Do I need to fill in anything here?
4. NAME, DATE AND PLACE OF BIRTH OF THE APPLICANT'S SPOUSE AND ALL NATURAL
CHILDREN...
- I am single without any children, what should be filled here?
For questions not applicable, do I write N/A in the answer column or leave it blank?

Given how INS documents generally go, I'd probably put "N/A" for 3 and "NONE" for 4.
However, maybe someone with specific knowledge of the DV lottery can jump in and
correct me in this case.

I found a very nice DV-2003 Green Card Lottery Application form with a sample entry
form showing how to fill. It is Free.

Let's look at the instructions. This is what it says: "There is no specific format
for the entry. Simply use a plain sheet of paper and type or clearly print in the
English (Roman) alphabet the following information. Failure to provide all of this
information will disqualify the applicant's entry."
Do you see where it says "... Failure to provide all of this information ..."? It
doesn't say anything about only writing the information that _apply_, but to provide
_all_ of this information. When something doesn't apply in your case, I suggest you
write N/A (not applicable). In that case you have at least provided _all_ of the
information.
