Extension of H1-B visa

My current H1-b visa is due to expire in 6 months time. The visa was issued 3 years ago
for a period of 3 years. I understand I can have my visa extended for an additional 3
years (for a total H1-b visa time of 6 years)

What do I, or my employer have to do to get my visa extended?
