Default ESTA - Are you known by any other names or aliases?

With regards to this question if you have gone a full name change and no longer use your former name would you select yes and enter your previous birth name?

On another note if you are married and legally changed your surname years ago would you select yes and enter your first and maiden name?

Yes and yes.

Both situations fulfill the requirements of the question.

Is the question "are you known?" Or is it "have you been known?". They mean different things. The first would be asking if you use any other names currently. Some professionals, such as doctors, and many entertainers, for example, maintain their maiden name for work after marriage. So a female doctor may be known as Dr Jane Doe professionally and publicly, but her passport may be in the name of Jane Smith.

You are splitting hairs. If you changed your name, whether by custom, deed poll, marriage, stage/pen name, or by any other method, the answer is "yes".

Obviously, the answer is yes to both questions. Those were names you were known by. All names and nicks are needed so that search processing can be done thoroughly.

And yes, hubby and I both listed our online names on the forms where indicated.
