Normally:

- H-1B approval notice
- your passport
- DS-156
- often also DS-157
- passport pictures
- evidence that you paid the visa application fee
- a letter from the employer stating that you are expected to work for
them
- some consulates also require copies of all documents that the employer
submitted to apply for the H-1B petition.
