Copy of DS-3025

Copy of DS-3025
Hi guys, finally moved to California last month 
and got married. Now have my SSN, driving licence, bank account etc sorted, however I sent off my vaccination documents to Dr Arnold (for my AOS) and in the rush never made a copy of the DS-3025 or put a return address on the envelope 
. Now it looks like this has gone missing so I'm panicking about how I'm going to get another copy of the DS-3025. I rang knightsbridge doctors and they said that they don't hold copies and that I should contact the embassy. Before I pay the ridiculous $16 fee 
for this call can anyone tell me if they have done this before or if they do hand out duplicates.
