Changing J-1 category from research to alien physian

Dear Visa Guru,

I have been working an institution for 2 years as a research fellow. I have J-1
visa with research category which was sponsored by University. I have been accepted
1-year preliminary residency program, and then applied to ECFMG for initial J-1
visa sponsorship and visa category change to alien physician. But the ECFMG refuses
to sponsor my application unless I offer the documentation for categorical spot. I
only got the 1-year preliminary residency. I have to apply again to categorical
spot in the end of this year. The ECFMG still refuse to sponsor my visa. If you
figured my problem and get the ECFMG sponsorship for the preliminary residency,
please let me know.
