Default change of position on H1 and I 485 issues

HI:
This is an urgent question and any help will be appreciated.
I am working as a clinic based Internal Medicine doctor on a H-1. I work in
the clinic and rotate through hospital service 6weeks a year.
Recently I was approached to take up full time hospitalist work by the same
hospital/medical group.
The work I do is the same, the time Ispend doing it changes from mostly
clinic and some hospital work to all or almost all hospital work.
My I-140 has been filed in april 2003 and my H-1 is valid till 11/05.
I just finished all the paperwork on the I485 and should be ready to submit
them shortly and attach to the already filed I140 as a joint application.
Now do I need to file a new Labour petition or is this new position should I
choose to accept it considered a new job with same employer or same job and
hence no need for changes to my paper work.
My job still needs the same skill set, the location is the same, the pay is
slightly higher, the employer is the same, hours almost same, major
difference is case load. Instead of dealing with clinic AND hospital
patients I will be dealing MOSTLy with hospital patients and very little
clinic
What do you folks think.

opinions.

Another Question: Can I run a Business on EAD or my wife on her H4 with EAD
obtained on the basis of NIW approval?

Finally , Ingo in one of the posts You had mentioned that a new I140 could
be substituted for an old one so that a pending I485 can be used.
My I485 has been pending for a long time almost 18months, though technically
it is not eligible for processing until I have completed 5yrs of underserved
area work. And Ihave done only 3 yrs at this point. After 3 yrs I had to
move to another job which is not underserved.
1.Can I substitute the old pending 485 instead of filing a new one
2.How do I do that? what is the legal regulation/rule supporting that.
3.If I substitute a new I140 for an old one.and since the old 485 is pending
for more than 180 days can I move to a same or similar job.
4. Will doing the above speed up the process of my getting GC . if so how
much time should I think about?
