I have a question with regards to me AOS, and the copy of the Birth Certificate that
is reqired.
I believe that I read somwhere that when you file for I-130 / I-48 that you require a
Birth Certificate that has details of your Parents Names, and Time of Birth, Address,
etc. is this correct.
I have a small birth certificate that is approx 7" Square that says :-
My Name : Sex : Date Of Birth : Registration District : Sub District :
I, .......... Registrar of Births And Deaths for the sib-district of ........... in
the Registration District of ........ do hereby certify that the above particulars
have been compiled from an entry in a register in my custody
Date ............ Signed ...................... NHS Number .......
Is this an acceptable form of birth certificate I am from the UK, and born in the
mid 1970's
is reqired.

No, that sounds like a short form birth certificate. You must get a long form birth
certificate with your parents' names. See
http://www.travel.state.gov/reciprocity/index.htm for U.K.
