Assembling the AOS application

just a few silly, nit-picky little questions we're stumbling on as we're putting this
mass of stuff together.
1. should we detach the instructions pages from the forms (485, 765, 864, 131) that
we're sending, or do they want the entire form intact?
2. how should we attach everything? we were going to use a two prong binder at the
top with dividers between each application, but that seems to be on the "things
not to do" list. so do we attach each bundle with binder clips or do we use
separate two-prong fasteners for each app? do we punch holes in the top of each no
matter what we use?
3. how do we attach the money orders? paper clip? binder clip?
4. what should go into our cover letter? just a statement that this is what we're
filing and here's what's enclosed, or should there be more to it than that? anyone
have samples posted anywhere?
i wish i could tell if it was a benefit or a hinderance to be this much of a
perfectionist about it all. :P

Can only tell you what we did and assure you that we had no problems with the applications/petitions being accepted.
We took each application/petition (w/o instructions) put the corresponding supporting documents behind each, put the check in front and binder clipped it if the stack was thick and paperclipped it if the stack was thin. Photos were marked on the back in pencil with the name and put in a small white envelope and labeled photo on the front.
The applications/petitions are separated eventually with the I-765 going to one department, the I-485 and I-864 to another and the I-131 to yet another so acco clipping them is a troublesome bother to the agent opening and dispatching your applications/petitions.
We did not include a cover letter but if you want one just put something like: "Enclosed for the purpose of adjustment of status from K-1 visa are:
1. I-485
2. I-864
3. Etc.
