Look for an attorney who is also a notary public. On their letterhead and with their notary seal, you should be fine.

Not yet. Have an appointment at our local office for 12 noon Wednesday February 6th. Just found out our local office closes at 12 noon on Wednesdays 
A phone call to check if I was given the correct information is needed.
After the saga with the check I sent to the UK end of last year, y'all be pleased to know I sent my pension claim form by DHL, it has currently cleared customs at East Midlands Airport.

There is a box on the form for the seal, no letterhead involved, they just witness the signature and check off on a list, what confirming document they saw, driving license or passport etc. and then sign over their seal or stamp.
