You may not have been asked for it, but you've always paid it. It's
included in the ticket fee by airlines that participate in the Visa
Waiver program. Travellers not using the Visa Waiver get ripped off.

Since it is an "inspection" fee, you will be charged regardless of your
citizenship. You come through US Immigration at a POE and undergo
inspection regardless whether you are a citizen, a permanent resident or a
visitor. This is my guess, though, so take it with a grain of salt.
