Getting healthcare back in UK when resident in US

This is not an imminent thing, I'm just curious. UK citizen, US permanent resident. If I wanted to get care back in the UK, how would this work? I assume I'd pay for it since I'm no longer a UK resident. Obviously there is also the cost of the flight to factor in. But has anyone been in this position?

A lot of people who were living 'under the radar' in EU countries were doing this, but after leaving the EU they have no where to hide, yes, you would have to pay, even in NHS hospitals, it would be up to you to work out whether it would be cheaper than getting the treatment in the US. Would any of your family be coming with you, that would be an extra cost, and of course this would only be for elective treatment, a special repatriation transfer would be horrendously expensive if you were non ambulant.
